Patients with progressive disease or older adults with frailty and their family members generally have poor awareness about the prognosis of their illness, and often fail to recognise the need to plan for future care. Without sufficient preparation, evidence have shown that conflicts between healthcare providers and families or among family members regarding the appropriate care resulted in situations of health crisis. These tensions arise because most of the family members have poor understanding of available care options and were unclear about the care preferences of their sick relatives. This project is a cross-sectoral collaboration designed to equip health and social care providers with knowledge and skills to conduct Advance Caring Planning. It can help patients and their family members determine the appropriate care options.
